Overview

Primary rhinoplasty reshapes the nose using open or closed approaches. Modern practice is structural and preservation-oriented: dorsal lines are refined, the tip is supported with grafts or sutures, and the airway is respected throughout.

What it changes

  • Dorsal height and profile line
  • Tip projection, rotation, and definition
  • Nostril shape and alar width
  • Bony vault width
  • Internal airway dimensions

Common goals

Reduce a dorsal hump
Refine a bulbous or drooping tip
Straighten a deviated nose
Narrow the alar base

Candidates

General factors a surgeon may consider. Only a qualified surgeon can assess whether you are a candidate.

  • General physical health suitable for the anaesthetic required
  • Realistic, clearly articulated aesthetic or functional goals
  • Stable anatomy appropriate for the technique proposed
  • Non-smoking or willingness to stop for the surgeon's required window

Surgical approach

  1. 01Open approach via a columellar incision, or fully endonasal
  2. 02Dorsal reduction or preservation, osteotomies for width
  3. 03Cartilage grafting from septum, ear, or rib for structural support

Recovery timeline

  1. Day 1

Splint in place, congestion, minimal pain for most people.

  2. Week 1

Splint removed around day 6–8. Bruising around the eyes peaks then fades.

  3. Week 2

Presentable for most social settings; nose still swollen, especially the tip.

  4. Week 4

Roughly 60–70% of swelling resolved; light exercise typically permitted.

  5. Month 3

Dorsum looks settled; tip remains the slowest area.

  6. Month 6

Definition emerges as tip swelling resolves.

  7. Month 12+

Final result — thick-skinned noses may take 18–24 months.

Risks & complications

Persistent or new breathing difficulty
Asymmetry, irregularity, or visible graft edges
Revision rate commonly quoted between 5% and 15%
Prolonged tip swelling, especially with thick skin
Numbness of the nasal tip
Infection or, rarely, skin necrosis

Limitations

Skin thickness limits achievable definition
Cannot correct an underlying skeletal or bite discrepancy
Small changes in bone/cartilage may not translate to visible change under thick skin
